Appointments
Your appointment time is reserved just for you. If you need to cancel, please give at least 24 hours’ notice.
• First missed appointment: explanation accepted
• Second missed appointment: $25 fee
• Third missed appointment: referral out of the practice
Insurance
We work with patients to help maximize insurance benefits. Because plans vary, we can provide an estimate but cannot guarantee coverage. Your estimated portion is due at the time of service.
We will submit insurance claims on your behalf and allow 45 days for payment. After 60 days, any remaining balance becomes your responsibility. Our staff is always happy to answer questions.
